Transcranial Doppler is a noninvasive ultrasound study that measures blood-flow velocity in major intracranial arteries. Low-frequency ultrasound at 2 MHz or below passes through relatively thin bone windows to insonate the basal cerebral arteries. Received echoes are processed into a velocity waveform with peak systolic and end-diastolic values. The frequently used transtemporal window can insonate the middle, anterior and posterior cerebral arteries and terminal internal carotid artery.
